Application Process
Thanks for your interest in Milpitas Christian School. Applying to a new school is an important decision. Below are the steps needed to apply to our Preschool, Elementary, and Middle School.

Preschool Process

STEP ONE: Complete online preschool application

STEP TWO: Complete enrollment forms
- Parents’ Rights Form
- Personal Rights Form
- Child's Preadmission Health History – Parent's Report
- Physician's Report
- TB Risk Assessment
The following documents are also required before your child can start school:- Birth Certificate or Passport (copy)
- Current Immunization Record (copy)

STEP THREE: Sign enrollment contract & pay $150 non-refundable enrollment fee
You’ll receive an email with specific class and start date information after your signed enrollment contract is received.

STEP ONE: Complete online application and pay $150 application fee
Application Fee: $150 Application Fee (Non-Refundable) - This fee is required to begin the Admissions process.

Elementary Documents (TK-5th Grade)- Birth Certificate

- Health Examination Form
- Report Cards
- Standardized Test Results (2nd Grade onward)
- Teacher Recommendation
- Student Support Documentation, IEP or 504 Plan (if applicable)
Middle School Documents (6th - 8th Grade)- Birth Certificate
- Immunization Record
- TB Risk Assessment
- Health Examination Form
- Report Cards
- Standardized Test Results
- General Teacher Recommendation
- Math Teacher Recommendation
- Student Support Documentation, IEP or 504 Plan (if applicable)

STEP THREE: Schedule Entrance Assessment or Placement Test
All incoming students are required to take the Entrance Assessment as part of completing the application for Milpitas Christian School. All entrance assessments are administered individually or in a group setting. We will contact you directly to schedule the assessment. -
STEP FOUR: Family Interviews
After all of your application documents have been received, Milpitas Christian School will contact you to schedule a family interview if needed.
