Resource Center Registration

Contract Agreement

By signing and submitting this form, you are agreeing to the terms of our tutoring program as described below.

1. Group tutoring sessions with the Academic Support Program are 45 minutes in length, twice per week, and are designed to build instructional and foundational skills and provide support.

2. Group tutoring sessions are contracted for the full year.

3. One-on-one tutoring sessions are 45 minutes in length and are designed to meet the specific instructional and academic needs of the individual student.

4. One-on-one tutoring sessions must be a recommendation from a teacher or the Resource Center. 

  

5. Tutoring sessions are scheduled by the Director of the Resource Center.

 

6. For one-on-one tutoring, thirty (30) day notice, in advance, is required to terminate the contract. The student account will be adjusted based on the actual number of scheduled sessions.

 

7. For one-on-one tutoring, thirty (30) day notice will be given when the student is ready to graduate from the Resource Center with account adjustments applied.

 

8. Tutoring reports will be issued weekly. Parent conferences are scheduled upon request.

 

9. For one-on-one tutoring, sessions missed due to illness will be made up with notification to the Resource Center by 7:30 AM the day of the illness. Failure to notify the head of the Resource Center (kyao@milpitaschristian.org) before 7:30 AM the day of absence will result in an unexcused absence and the session will be billed.  Missed sessions due to school functions will also be made up. 

 

10. For one-on-one tutoring, student accounts will be billed for unexcused absence from a session due to family vacation, medical/dental appointments, detentions, suspensions, or non-MCS related activities or non-emergency conflicts.

11. Equal monthly fees will be billed to your account through May.
